There's a section to download the proposal form. It definitely is something that takes several people to support - so you definitely want to make sure you have the community to help out. What's great about the application (IMHO) is that it walks you through planning the entire chapter and really makes you think about just *how* you're going to do it, from establishing goals to who will help with what to even thinking about where you could possibly perform. The proposal is really a great tool designed to help you figure out if you can set up a chapter - if you'd like to do so I'd definitely recommend taking a look at it and seeing what you can do.
